Steps To Take When Opening a New ESRD Facility in Texas
Step 1.
Contact the Department of State Health Services (DSHS) for an "ESRD Facility Application Packet".

The DSHS packet has to be requested in writing via fax.
Your request must include:
- A statement that this is a request for an "ESRD Application Packet".
- Your name or whom DSHS needs to contact concerning the new facility, plus a phone number.
- Mailing address for application packet.
Step 2.
At Least 6 weeks before clinic opening:
Download the following information and fax to the ESRD Network of Texas Inc. (#14) to the attention of Debbie O’Daniel. Fax: 972-503-3219 or email info@nw14.esrd.net.
