Opening a New Facility

Please download and print the complete packet and forms. Welcome letter includes instructions for completing and submitting forms. Please read entire packet prior to completing the forms.

Make sure forms are submitted at least 6 weeks prior to DSHS survey or approximate open date of facility. Completed forms should be returned to the Network office via fax at 972-503-3219.
