Fistula First Data Collection Software

A major component of the Fistula First project is the collection of prevalent and incident VA utilization data from outpatient dialysis facilities to evaluate trends in AVF placement. The Large Dialysis Organizations (LDOs – Gambro, RCG, DaVita, DCI and Fresenius) have collaborated with CMS to develop an automated process that forwards monthly facility VA utilization data electronically to the ESRD Networks. Currently, non-LDOs are providing qurterly VA utilization data via CMS Data Collection Software (see descriptions below).

Fistula First Data Collection Software

Per CMS request, all independent facilities and small local dialysis corporations are to utilize this software to electronically document and report quarterly facility specific vascular access rates to their Network office via e-mail or fax.
